NURSING HOME TRANSITION AND DIVERSION (NHTD)
At Safe and Prudent Homecare, we believe that every individual—whether living with a
disability or aging gracefully—deserves the dignity of choice, independence, and the ability to
live life on their own terms. Our services, offered through the Nursing Home Transition and
Diversion (NHTD) Medicaid Waiver Program, reflect this commitment to personal
empowerment and community inclusion.
This program is rooted in the understanding that everyone has the right to make decisions, take
risks, and learn from life’s experiences, all while receiving the support necessary to ensure
health, safety, and well-being. Every care plan we develop is as unique as the individual it
serves, tailored to reflect each participant’s strengths, needs, goals, and preferences.
Our collaborative approach puts you at the center of every decision. Working closely with our
compassionate providers, you will help shape a personalized Service Plan designed to promote
independence, self-reliance, and meaningful engagement with your community.

Eligibility Requirements
To participate in the NHTD Waiver Program, you must:
1. Be eligible for Medicaid coverage that supports community-based long-term care.
2. Have acquired a physical disability after the age of 18.
3. Be assessed as requiring a nursing home level of care, as determined by the Hospital and
Community Patient Review Instrument (H/C PRI) and SCREEN assessments.
4. Be able to live safely in the community, as determined by the Regional Resource
Development Specialist (RRDS).
We understand that transitions can be challenging, whether you’re seeking to return to your
community after time in a nursing facility or looking for preventive support to avoid
institutionalization. Our clients include individuals who were once independent professionals
whose lives have been unexpectedly changed by illness or injury.
